Life Care Planning
Life Care Planning is a comprehensive, team-based service designed to help older adults and their families navigate the legal, financial, and healthcare challenges of aging — all under one coordinated plan.
At Everbright Legacy Law, our Life Care Planning model brings together an attorney, elder care social worker, and paralegal to provide ongoing support focused on three core goals:
- Protecting and preserving assets
- Securing appropriate and quality care
- Accessing government benefits such as Medicaid and VA programs
This service is offered as a year-long engagement, with the option to renew annually* based on your continuing needs and evolving circumstances.

What Life Care Planning Includes
Reviewing and updating wills, trusts, powers of attorney, and healthcare directives.
Developing a custom asset preservation strategy based on your goals and resources.
Coordinating access to long-term care services, whether at home, in assisted living, or in a nursing facility.
Applying for and maintaining eligibility for Medicaid (Medical Assistance), Veterans Benefits, and other government programs.
Providing ongoing guidance and advocacy throughout the year, with regular check-ins and support for emerging issues
Unlike traditional one-time legal services, Life Care Planning is a dynamic relationship. As your or your loved one’s needs change, we adjust the plan accordingly — whether that means updating legal documents, revisiting care options, or applying for public benefits.

Who Benefits from Life Care Planning?
Life Care Planning is ideal for:
Older adults beginning to experience health or cognitive changes
Spouses or adult children responsible for managing care
Families concerned about the cost of nursing home care
Those needing help navigating both the legal and care systems together
It’s especially valuable when your situation involves complex family dynamics, special needs, or ongoing decision-making around health and finances.

- Scheduled reviews with your attorney to update your estate plan as necessary and review your finances and cash flow to ensure your plan remains aligned with your goals.
- Preparation and timely submission of renewals for government benefits, including Medicaid and VA Aid & Attendance, to maintain eligibility and maximize support.
